Telehealth Consent

TELEHEALTH INFORMED CONSENT
GoodMDs, Inc. and GoodMDs Medical Group

Please read this consent carefully. You will be asked to agree to it before you submit a visit request. If you have questions, call us at 1-888-613-7454 or email order@goodmds.com before you continue.

What telehealth is

Telehealth means getting health care from a licensed provider who is not in the same place as you. At Good MDs, you answer a medical questionnaire online. A provider licensed in the state where you are located reviews your answers and may contact you by phone, text or email. The provider may then recommend treatment, send a prescription to the pharmacy you choose, order lab tests, or advise you to get care in person.

Much of our care is “asynchronous”: the provider may make a decision from your written answers without a live call. The provider can always ask for a phone call, more information or a lab test before deciding.

Expected benefits

  • Care without traveling to an office or waiting room.
  • Faster access to a licensed provider, usually within hours during business hours.
  • Prescriptions sent to a pharmacy near you, when appropriate.

Possible risks and limits

  • The provider cannot examine you in person. Some conditions cannot be diagnosed or treated safely without a physical exam or tests, and the provider may refer you for in-person care.
  • The provider relies on the information you give. Incomplete or inaccurate answers can lead to a wrong diagnosis or unsafe treatment.
  • Technical problems (for example a lost connection or an email that does not arrive) can delay your care.
  • Although we use security measures to protect your information, no electronic system is completely secure, and information could be accessed by others despite these measures.
  • A prescription is never guaranteed. The provider decides what is medically appropriate, and some medicines (including controlled substances) can only be prescribed after an in-person visit or under specific federal and state rules.

Not for emergencies

Good MDs does not provide emergency care. If you have chest pain, trouble breathing, signs of a stroke, severe bleeding, thoughts of harming yourself or others, or any other emergency, call 911 or go to the nearest emergency room. For a mental health crisis, call or text 988.

Your responsibilities

  • Give complete and truthful answers about your health, medicines, allergies and identity.
  • Be located in a state where Good MDs provides care at the time of your visit, and tell us your correct location.
  • Read and follow the instructions your provider and pharmacist give you, and tell us or another provider if your symptoms get worse or do not improve.
  • Follow up with your primary care provider or a specialist when advised.

Minors

If the patient is under 18, a parent or legal guardian must complete the request, give consent on the patient’s behalf and provide their own details. Some treatments are not available to minors.

Partner treatment

If you request treatment for a partner, you confirm that your partner has agreed to it, that the information you give about them is accurate, and that you will share the provider’s instructions with them. The provider may decline partner treatment.

Privacy and records

Your answers and the provider’s notes become part of your medical record. We keep and protect them as described in our Notice of Privacy Practices and Privacy Policy. You can ask for a copy of your records.

Communication

By giving your phone number and email address, you agree that Good MDs and its providers may contact you by phone, text message and email about your visit, prescriptions, lab results and follow-up. Message and data rates may apply. Please do not email sensitive medical details; we will ask for them in a secure way.

Fees

The visit fee and any add-ons are shown before you submit. Good MDs does not bill insurance for visits. Medicines and lab tests may be paid separately at the pharmacy or lab, where your insurance may apply.

Your right to refuse or withdraw

You may refuse telehealth care, stop at any time, or ask for in-person care instead. Withdrawing your consent does not affect care you already received. To withdraw, contact us using the details below.
